What a lot of patients don\u2019t realize is that if they\u2019ve worn their front teeth down so much, they can do a couple of different things.<\/p>\n
Number one, if the front teeth are worn down, the back teeth are more than likely worn down because there\u2019s more force that\u2019s generated the further back that you go as opposed to in the front. There\u2019s no question that a lot of damage has been done on the back teeth as well. If that\u2019s the case, in some cases, the patient\u2019s actually lose what\u2019s called vertical dimension.<\/span><\/p>\n Vertical dimension means that if they have a certain height when they\u2019re born, or if the teeth haven\u2019t been damaged or worn down, they have a certain height. That height dictates the soft tissue. Your soft tissue really includes the bottom third of your face from underneath the nose to underneath the chin.<\/p>\n For patients who have lost significant vertical dimension, you might notice them because their lips have become flattened or their mouth, when it\u2019s closed, is wider than most people\u2019s, or their cheeks might be a little bit sunken in. Those are signs of a patient who has aggressively worn down the teeth and lost vertical dimension.<\/p>\n If you\u2019ve lost two or three millimeters, sometimes that\u2019s enough to see a physical outward appearance, or change in the lip line or the cheeks or just the whole measurement from beneath the nose to beneath the chin. In that case, you are talking about restoring every single tooth, top and bottom, to build back that dimension that\u2019s been lost.<\/p>\n In another case, or in that same case, another concern of mine, is when the teeth are worn down so aggressively you can start to develop pretty significant sensitivity, because underneath the enamel is dentin. Dentin is much softer than enamel and wears about 50 times faster.<\/p>\n You can imagine how long it would take to wear through the enamel but if you\u2019re grinding so aggressively and quickly through the enamel, imagine once you get through the enamel and you\u2019re into dentin, it\u2019s going to wear quite a bit faster and you\u2019re going to start to develop sensitivity.<\/p>\n Sometimes I\u2019ve seen patients that have worn so far through their dentin they are actually wearing into the nerve of the tooth. There\u2019s a wide range there, but ultimately, it\u2019s really, really important that you assess the whole picture.<\/p>\n","protected":false},"excerpt":{"rendered":"
